DOT physicals

Performed by Suleman Aziz, MD, a certified medical examiner listed on the FMCSA National Registry.
For commercial drivers; fee is $150. DOT exams are usually not covered by insurance.
Bring photo ID or driver’s license, glasses or contacts or hearing aids, and a medication list with prescriber names.
If applicable, bring cardiology clearance, a CPAP compliance report covering at least 90 days, a recent A1C, and Form MCSA-5870 if you use insulin.
The urine test is a kidney screen, not a drug test.
Depending on results, the examiner may issue a shorter certificate, request more information, or be unable to certify you.
